- How you make a difference
- The Patient Enrollment Coordinator works alongside Discharge Planners, facility leadership, and local county agencies to support Medi-Cal coverage verification, enrollment activities, and Justice-Involved Reentry Initiative processes for individuals in custody.
This role verifies existing Medi-Cal coverage, Client Identification Numbers, and managed care plan assignments and assists with enrollment activities when coverage is not active. The position coordinates with county agencies regarding enrollment determinations and manages applicable Justice-Involved aid code activation and deactivation through the JI Reentry Portal. Eligibility and enrollment determinations are made by the appropriate county social services or welfare agency.
- Key Responsibilities
- + Verify Medi-Cal coverage, Client Identification Numbers, and managed care plan assignments using approved systems and coordinate with county agencies to obtain required information.
+ Assist individuals with Medi-Cal applications, renewals, and enrollment documentation while tracking applications and county determinations.
+ Document coverage status, enrollment information, managed care plan assignments, and related activities in designated systems.
+ Activate and deactivate applicable Justice-Involved aid codes through the JI Reentry Portal in accordance with established procedures.
+ Collaborate with Discharge Planners and other care team members, maintain accurate records, and escalate enrollment or system issues to the appropriate contact.

- Qualifications & Requirements
- Education + High school diploma or equivalent is required.
+ Additional education or training in healthcare administration, social services, eligibility/enrollment services, or a related field is preferred. Experience
+ Experience in a correctional facility or healthcare environment preferred.
+ Experience in a healthcare, correctional, social services, benefits enrollment, eligibility verification, or administrative environment is preferred.
+ Experience working with Medi-Cal, Medicaid, public benefits programs, healthcare eligibility systems, or enrollment processes is preferred.
+ Experience working with electronic healthcare, eligibility, or case management systems is preferred. Licenses/Certifications
+ Must maintain CPR certification, and any other certifications (such as First Aid) required by contract. This position is available only to those who reside in the United States.
