This position is based on traditional Wraparound principles but is specifically tailored to supporting adults with developmental disabilities. The Clinical Facilitator will provide wraparound care coordination and clinical facilitation, helping clients navigate multiple systems and ensuring that services are coordinated around the individual's needs, strengths, and goals. This position will work collaboratively with clients, families, behavioral health providers, social services, court systems, community agencies, and other professionals involved in the client's care. The Clinical Facilitator will work closely with individuals, families, caregivers, community providers, Regional Center staff, and other members of the individual's support team to ensure services are coordinated and focused on the individual's goals, independence, and quality of life.

Responsibilities Care Coordination:

Develop and oversee individualized care plans, connecting families with necessary services and community support.

Team Facilitation:

Team meetings, ensuring active participation and shared decision- making.

Family Engagement:

Build strong, trusting relationships, empower families, and ensure their perspectives drive the process.

Assessment and Monitoring:

Conduct strengths-based assessments, track progress, and adapt plans as needed.

Documentation and Reporting:

Maintain detailed records for service authorization, billing, and evaluation.

Crisis Support:

Provide or arrange for crisis intervention as part of the overall plan. Qualifications Master's degree in Social Work, Marriage and Family Therapy, Counseling, Psychology, or another closely related clinical field. Bilingual English/Spanish proficiency required. Experience working with adults and/or families experiencing behavioral health or complex social needs. Strong understanding of community resources and social service systems. Ability to coordinate services across multiple providers and systems. Strong communication, organization, documentation, and interpersonal skills. Ability to work independently while collaborating effectively with multidisciplinary teams.

Pay:

$64,521.60 - $77,703.43 per year
