Job Description
Key ResponsibilitiesClient Care & Relationship Management Conduct initial and ongoing client assessments to understand each client's needs, preferences, routines, and goals. Develop and maintain individualized care plans. Maintain regular communication with clients and their families to ensure satisfaction and address changing needs. Conduct regular client check-ins and in-home visits as appropriate. Monitor client condition and identify changes that may require adjustments to the care plan. Serve as a trusted resource for families navigating the challenges of aging and in-home care. Respond promptly to client and family concerns and follow through until issues are resolved. Build long-term relationships with clients and their families. Caregiver Management Match caregivers with clients based on skills, experience, personality, availability, and client preferences. Communicate care expectations and client-specific needs clearly to caregivers. Monitor caregiver performance, attendance, punctuality, and client feedback. Provide coaching, guidance, and support to caregivers. Address caregiver performance or reliability issues promptly and professionally. Recognize and communicate outstanding caregiver performance. Help maintain strong caregiver-client relationships and continuity of care. Scheduling & Operations Assist with coordinating caregiver schedules and ensuring client shifts are properly staffed. Respond quickly to call-offs, schedule changes, and other urgent staffing needs. Coordinate coverage for new clients, changing schedules, hospital discharges, vacations, and other special circumstances. Maintain accurate client and caregiver records in the company's scheduling and care management systems. Ensure required documentation, care notes, and client communication are completed accurately and on time. Monitor service delivery and identify potential operational issues before they become problems. Care Coordination Communicate with family members, physicians, therapists, hospitals, senior living communities, and other professionals involved in a client's care when appropriate. Help coordinate transitions home following hospitalization, rehabilitation, or other care settings. Identify opportunities to improve client safety, independence, and quality of life. Participate in care conferences and other meetings when appropriate. Escalate significant changes in client condition or safety concerns appropriately. Customer Service & Growth Deliver an exceptional client experience that reflects HomeWell's commitment to personalized care. Identify opportunities to increase or modify services as client needs change. Support client retention by maintaining strong relationships and proactively addressing concerns. Build positive relationships with referral partners and community professionals. Represent HomeWell professionally at community events, meetings, and referral-partner visits when needed. Help educate families and referral partners about HomeWell's services and specialized programs. Qualifications Required:
Experience in home care, senior care, healthcare, case management, care coordination, or a related field. Strong communication and organizational skills. Ability to manage multiple clients and competing priorities. Valid driver's license, reliable transportation, and ability to travel throughout the service area. Ability to work occasional evenings, weekends, or after-hours as needed for urgent client or staffing situations.Preferred:
Bachelor's degree in nursing, social work, gerontology, healthcare administration, or a related field. RN, LVN, CNA, HHA, or other relevant healthcare/caregiving experience. Previous experience managing caregivers or coordinating home care services. Experience with home care scheduling or electronic care management software. Success in This Role A successful Care Manager will: Keep clients and families informed and confident in their care. Have relationships with companies in the industry in San Diego Build strong relationships with caregivers and clients. Resolve problems quickly and professionally. Maintain excellent documentation and follow-through. Help ensure shifts are properly staffed and clients receive consistent care. Identify changing client needs before they become emergencies. Contribute to excellent client retention and satisfaction. Represent HomeWell as a trusted, professional resource for families and referral partners.
