Role and Responsibilities:

This position involves coordinating resident assessments and care plans in collaboration with multiple departments, ensuring compliance with state regulations and organizational policies. Key duties include overseeing data collection, facilitating care plan conferences, monitoring assessments such as PPS, OBRA, RAPs, and Triggers, and maintaining accurate documentation. The role requires evaluating assessments, updating policies, transmitting MDS reports, and participating in surveys and inspections by government agencies. Additionally, the position may perform nursing functions and administrative tasks related to medical forms and evaluations. An ongoing quality assurance program is essential to ensure assessment accuracy and compliance.

Qualifications and Skills:

Current, unencumbered RN license in the state. Minimum of two years' managerial experience in healthcare settings preferred. Knowledge of age-appropriate care, medical practices, laws, and regulations. Strong problem-solving, communication, and customer service skills. Proficiency with office software such as Word, Outlook, and Excel. Flexibility to work rotating managerial on-call shifts.

Physical and Working Conditions:

Light physical effort, handling objects up to 20 pounds. Frequent standing, sitting, bending, and interactions with residents and families. Subject to interruptions and active communication with residents and team members.
