We are seeking an energetic, detail-oriented, and people-focused Patient Care Coordinator (PCC) to join our growing team. This is more than a front desk role—it's an opportunity to make a real impact on patient experiences and outcomes while working alongside some of the best therapists in Colorado. What You'll Do As a Patient Care Coordinator , you will: Provide exceptional service to every patient—greeting, scheduling, and supporting their rehab journey. Convert new referrals into scheduled patients and ensure treatment plans stay on track. Manage insurance authorizations, benefits verification, and co-pay/supply collections with accuracy. Coordinate daily operations with the Director of Clinical Operations and Director of Communications . Monitor patient care plans (POCs) and help ensure patients complete their prescribed visits. Track, analyze, and report key metrics to improve clinic efficiency and patient outcomes. Act as a communication hub between patients, therapists, and leadership. Contribute ideas, share priorities, and proactively flag roadblocks so the team can succeed together. Comfort with technology and data—tracking progress, running reports, and spotting trends. A proactive mindset—you take ownership, anticipate needs, and look for solutions. Qualifications Prior customer service and/or administrative experience (healthcare or office setting preferred). Strong communication skills—both in person and over the phone.
