What You'll Do:

Coordinate and complete accurate, timely MDS assessments as required by regulations and facility standards Collaborate with the interdisciplinary team to ensure care planning reflects resident needs and goals Maintain compliance with state and federal regulations related to MDS processes Contribute to quality improvement initiatives and help track outcomes

What You'll Need:

Current, active RN or LPN license in Colorado Prior experience as an MDS Coordinator in LTC/SNF Strong organizational and communication skills A team-first approach and commitment to excellence Be part of a close-knit team in a community you'll love. and help us continue building excellence at Glenwood Springs Health Care!
