This role ensures that residents receive high-quality, individualized care customized to their unique needs while supporting the broader care team across departments. The Assisted Living Coordinator coordinates closely with healthcare professionals, family members, and staff, and performs all delegated responsibilities in accordance with community Policies and Procedures and in compliance with all relevant rules and regulations. Duties and Responsibilities include, but are not limited to: Resident Care & Care Planning Develop and implement individualized care plans for residents in the community, ensuring plans reflect each resident's current needs, preferences, and goals. Conduct regular assessments of residents to identify changing needs and adjust care plans accordingly. Monitor medication management and ensure that residents receive their medications as prescribed. Maintain accurate and up-to-date records of resident care, assessments, and care plan updates. Coordinate with healthcare professionals to ensure residents receive appropriate medical care and support. Ensure that the community's policies and procedures are followed to provide safe and effective care to all residents. Family & Resident Communication Communicate regularly with family members and loved ones to keep them informed of residents' care, progress, and any changes in condition. Provide support and guidance to residents and their families as they navigate the healthcare system and community resources. Serve as a compassionate point of contact for residents and families, addressing concerns promptly and professionally. Staff Coordination & Training Supervise and support care staff to ensure consistent delivery of high-quality, person-centered care. Train care staff on individualized resident care plans, community standards, and relevant policies and procedures. Coordinate with other departments to ensure that all resident needs are addressed in a timely and comprehensive manner. Marketing & Move-In Coordination Work collaboratively with the Marketing Director to support new resident move-ins, including preparing care documentation, coordinating room readiness, and ensuring a smooth and welcoming transition for new residents and their families. Participate in move-in assessments and initial care plan development for incoming residents. Assist in communicating care capabilities and services to prospective residents and families as requested by the Marketing Director. Cross-Departmental & Leadership Support Perform duties and tasks as assigned by the Wellness Director, Staffing Manager, or Executive Director. Support staffing coordination efforts in collaboration with the Staffing Manager when care coverage needs arise. Attend and participate in scheduled leadership and care team meetings. Contribute to continuous quality improvement efforts related to resident care and departmental processes. Perform other duties as assigned in accordance with community Policies and Procedures. Qualifications and Experience Minimum High School Diploma or GED preferred; additional education or certification in nursing, healthcare, or senior living a plus. Prior experience in a senior living, assisted living, or healthcare setting preferred. Excellent communication and interpersonal skills with residents, families, and interdisciplinary teams. Ability to work collaboratively with a diverse team of healthcare professionals. Knowledge of regulations and best practices related to senior and assisted living care. Strong organizational skills and attention to detail. Ability to prioritize and manage multiple tasks effectively in a fast-paced environment. Compassionate and patient demeanor with a genuine commitment to serving seniors. Skills Required Ability to maintain composure in stressful or fast-changing situations. Resourcefulness and strong problem-solving skills. Ability to make informed decisions and accept responsibility for outcomes. Strong organizational and time management skills with the ability to meet deadlines. Ability to train, guide, and motivate care staff effectively. Willingness and ability to serve as a role model in professional conduct and appearance. Evidence of effective and constructive problem resolution skills. Basic computer skills for documentation, scheduling, and communication. Working Conditions The Assisted Living Coordinator works in a senior living community and may be exposed to infectious waste, diseases, and other conditions inherent to a healthcare environment. Personal Protective Equipment (PPE) is required and will be provided. This position may require working evenings, weekends, and holidays as needed. The work environment is often fast-paced and requires the ability to respond quickly to emergencies or changing resident needs. The Assisted Living Coordinator is a critical member of the care team and plays an important role in ensuring that residents receive high-quality, individualized care every day. Physical Requirements Must be able to lift and move over 30 pounds. Ability to sit, stand, bend, lift, push, pull, stoop, walk, and reach intermittently during working hours. Ability to move at a high pace for extended periods of time. Frequent physical tasks include assisting residents with mobility and daily activities. Reasonable accommodation may be made in accordance with ADA. Hours and Dress Code Non-exempt, hourly position. Must be available on weekends and as needed for evenings or holidays to meet resident care needs. Uniforms as provided by the community.
