Behavioral Health Care Manager Reports To:
Director of Clinical Operations Summary:
In combination with the patient's healthcare team, the Behavioral Health Care Manager will work with our behavioral health, high-risk and medically complex population to increase compliance, connect patients to resources, educate and support patients. Effectiveness should increase patient satisfaction, increase health, decrease health risks, and reduce acute episodes of care.Responsibilities:
Collaborate in overseeing the practice's behavioral health care management program Work with patients to establish trust and ensure compliance. Develop a comprehensive, collaborative care plan to support the provider's treatment plan. Follow-up on treatment goals including medication management, support with family members, and specific therapeutic assignments to work on between sessions. Triage changes in the patient's status appropriately with the care team. Crisis planning, crisis management and safety planning for patients at risk of harm or self-harm. Assist with and facilitate the transition of care from inpatient and outpatient settings such as hospital, rehabilitation facilities, and crisis center to home. Identify and work to resolve social determinants of health affecting each patient such as employment, transportation, advocacy, legal aid, housing, food assistance and financial aid. Develop relationships with other caregivers and organizations in the community and communicate with them regarding patient needs frequently. Provide individual patient or family education and self-management support that is appropriate based on the patient and health diagnoses. Address, educate, and reconcile quality measures such as depression screenings or other Behavioral Health screenings used within the organization. Maintain compliance with HIPAA.Requirements:
Must be experienced in a physician's office or equivalent combination of training and experience. Must have experience with medical documentation, and EMRs. Must have knowledge of examination, diagnostic and treatment room procedures. Must have knowledge of medical equipment and instruments. Must have knowledge of common safety hazards and precautions. Must be able to multi-task. Must be punctual, meticulous, and reliable. Must be able to maintain strict confidentiality. Must be able to stand for extended periods of time. Must be able to occasionally lift and carry up to 20 pounds. Must be a Team player (maintain respectful co-worker relationships). Skill in identifying and resolving problems. Skill in maintaining clinical quality assurance. Ability to interpret, adapt and apply guidelines and procedures. Ability to use good reasoning and judgment and react calmly in emergency situations. Ability to establish and maintain effective working relationships with patients, medical staff, coworkers, and the public. Ability to read, write and communicate effectively orally and in writing. Ability to maintain records and record test results.Education:
Bachelor's Degree Experience:
Preferred to have experience in a medical office, nursing home, or inpatient facility. Experience in behavioral health management is preferred.Mental and Physical Requirements:
Varied activities including standing, walking, reaching, bending, and lifting. This position requires a full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. Must be able to use a variety of office equipment. May require working under stressful conditions.Conditions:
Normal office and exam room environment. The position requires the ability to work under pressure and with a diverse population, including staff, physicians, clients, patients, insurance companies, labs, hospitals, and other members of the public on a regular basis.
