WCS Healthcare Partners is seeking a Bilingual Medical Care Coordinator to support a Long Term Foster Care program. This position plays a vital role in ensuring children receive timely, coordinated medical care by managing appointments, maintaining accurate medical records, and collaborating with healthcare providers and the interdisciplinary care team. This is a full-time, daytime opportunity. In this role, you will: Coordinate medical care from initial assessments through scheduling, follow-up, and resolution to ensure seamless care transitions. Schedule and align appointments across patients, nurses, and physicians to maintain efficient operations. Verify information in medical records to ensure accuracy for the care team. Follow up on open medical needs until resolution is achieved. Ensure all required forms and documents are completed correctly and on time. Regularly check in on patients and document their care progress. Provide administrative support to maintain the effective running of clinical services. Location This full-time daytime position is based in New Haven, CT, a vibrant city known for its rich history, cultural attractions, and close-knit community. Required Qualifications Bachelor's degree in public health or a related field. At least one year of experience in a healthcare setting. Fluency in English and Spanish, spoken and written.
