Job Description Job Description We are looking for a caring, organized, and dependable Patient Care Coordinator to work directly with residents in an assisted living facility in Port Charlotte, FL area. The Care Coordinator will help residents stay connected with their healthcare team, assist with daily routine health monitoring of vitals, follow up on ongoing medical needs, and help make sure important changes in a resident's health are communicated to the appropriate provider, alf staff and families. Key Responsibilities Build positive relationships with residents and become a familiar point of contact for their healthcare needs. Speak with residents and families about available health support services and help them participate. Assist residents with simple health monitoring such as blood pressure, weight, oxygen level, or blood sugar readings. Review incoming health information and alert the medical team when readings or symptoms require attention. Check in with residents regarding medications, appointments, recent hospital or emergency room visits, and changes in their health. Help coordinate physician visits, telehealth appointments, laboratory testing, and other follow-up care. Communicate with physicians, nurse practitioners, facility staff, residents, and families as appropriate. Help residents stay on track with their treatment plans and recommended follow-up. Document resident conversations, health updates, concerns, and follow-up activities accurately. Identify residents who may need additional attention and communicate concerns promptly to the clinical team. Serve as a liaison between the assisted living facility and the medical practice. Ideal Candidate We are looking for someone who is: Friendly, patient, and comfortable working with seniors. Confident speaking with residents and families. Organized and good at following up. Comfortable using computers, tablets, and basic healthcare technology. Dependable and able to work independently. Comfortable communicating with doctors, nurses, facility staff, and families. Someone who takes ownership and makes sure important tasks do not fall through the cracks. Preferred Experience Healthcare experience is highly preferred.
