Job Description

HCA Healthcare is seeking a Home Health Clinical Manager to lead clinical operations for our home health services. In this role, you will oversee patient care delivery, supervise nursing and allied health staff, ensure regulatory and accreditation compliance, and coordinate interdisciplinary care plans. You will monitor quality, patient outcomes, and documentation, using data to drive improvements. The Clinical Manager partners with physicians, patients, and families to support safe transitions of care, manages staffing and scheduling, and mentors clinicians, fostering a culture of compassion, teamwork, and continuous learning. Responsibilities Lead day-to-day clinical operations for home health services Supervise, coach, and evaluate nursing and allied health staff Ensure compliance with regulatory, accreditation, and payer requirements Oversee development and execution of individualized care plans Monitor quality metrics, patient outcomes, and clinical documentation Coordinate interdisciplinary care and safe transitions between settings Manage staffing levels, scheduling, and caseload assignments Collaborate with physicians, patients, and families on care decisions Implement quality improvement and evidence-based practice initiatives Support staff education, onboarding, and professional development Required Skills Home health regulations and compliance Clinical care management Staff supervision and leadership OASIS assessment and documentation Care coordination and discharge planning Quality improvement and outcome measurement Electronic health record (EHR) proficiency Policy and procedure development Data analysis and reporting Interdisciplinary team collaboration
