Job Description

Position Summary The Patient Care Coordinator is responsible for coordinating the daily operations of a physician who provides primary care services in assisted living and independent living facilities. This role serves as the primary administrative liaison between the physician, facilities, patients, families, pharmacies, home health agencies, diagnostic companies, and durable medical equipment (DME) suppliers. The ideal candidate is highly organized, proactive, able to multitask, and comfortable communicating with multiple healthcare partners throughout the day.
Essential Responsibilities:
Scheduling & Facility Coordination Communicate daily with assisted living facilities to obtain patient schedules. Coordinate physician schedules with multiple facilities. Contact independent living patients to confirm appointments. Manage schedule changes and communicate updates promptly. Ensure patient information is accurate before each visit. Insurance & Eligibility Verify insurance eligibility and benefits prior to appointments. Identify authorization requirements when applicable. Resolve insurance-related issues before patient visits whenever possible. Clinical Coordination Process physician orders for: Laboratory testing Mobile X-rays Urinalysis Other diagnostic testing Coordinate DME orders and referrals. Submit referrals to specialists as directed. Track outstanding orders until completed. Documentation & Communication Fax physician notes to: Home Health agencies Assisted living facilities Other healthcare providers as needed Update patient pharmacy information in the electronic medical record. Upload and organize medical records. Maintain accurate documentation of all communications. Patient & Family Communication Serve as a point of contact for patients and family members regarding administrative questions. Communicate appointment information and follow-up instructions. Coordinate non-clinical requests between families and the physician.
Care Coordination Work closely with:
Assisted Living Facility staff Nurses Home Health agencies Pharmacies Diagnostic companies DME vendors Follow up on outstanding tasks to ensure timely completion. Escalate urgent concerns to the physician as appropriate. Qualifications Medical Assistant, Healthcare Administration, Medical Office, or related healthcare experience preferred.
Previous experience in:
Primary Care Home Health Assisted Living Internal Medicine Care Coordination Knowledge of medical terminology. Experience with electronic medical records (EMR). Familiarity with insurance verification and referrals preferred. Skills Exceptional organizational skills Strong multitasking ability Excellent verbal and written communication Ability to prioritize competing demands Comfortable making frequent phone calls Strong customer service skills Detail-oriented Ability to work independently Problem-solving mindset
Pay:
