Supportive management, cohesive work environment and many different career paths to help you achieve your professional goals Summary Of the Care Manager The Care Manager will be responsible for developing and maintaining strong referral relationships with hospitals, physicians, case managers, assisted living facilities, and community partners to help grow admissions and strengthen the facility's presence in the community. Requirements/Qualifications of the
Care Manager:
Previous healthcare marketing or admissions experience preferred 2 years of experience in Skilled Nursing Facility or healthcare experience required Excellent communication and interpersonal skills Strong organizational and follow-up abilities Ability to work independently and manage relationships professionally Reliable transportation required for local travel Knowledge of Medicare, Medicaid, and managed care preferred
Responsibilities Of The Care Manger:
Build and maintain referral relationships with hospitals, healthcare providers, and community organizations Conduct onsite visits to hospitals and referral sources Coordinate admissions referrals with the clinical and admissions teams Educate referral partners on facility services, specialty programs, and clinical capabilities Track referral activity and admissions trends Participate in community events, networking opportunities, and marketing initiatives Ensure excellent communication between referral sources, residents, and families Collaborate with facility leadership to meet census and occupancy goals
