Full-Time Lead. Develop. Build Relationships. Drive Growth. Preferred Care Home Health is seeking an experienced clinical leader to manage and grow our Senior Living Programs across the SWFL market. This is a hands-on leadership opportunity for an RN, PT, or OT who understands home health, knows how to develop clinical teams, and enjoys building strong relationships with senior living communities, referral sources, and healthcare partners. The ideal candidate is a people-focused, relationship-driven, and growth-minded leader who can balance clinical oversight, operational performance, team development, and business growth. What We're Looking For Active Florida RN, PT, or OT license Minimum 5 years of medicare home health experience Previous leadership or management experience in home health, healthcare, senior living, or a related setting Experience as a Clinical Manager, Clinical Supervisor, Director, Therapy Manager, Program Manager, or similar leadership role Strong ability to lead, coach, develop, and hold clinical teams accountable Experience working with senior living communities, referral sources, physicians, clinicians, and healthcare partners Strong understanding of home health operations, Oasis Documentation, clinical care, care coordination, and therapy services Ability to monitor program performance, identify opportunities, and implement process improvements Strong communication, organization, problem-solving, and relationship-building skills A proactive, collaborative, and growth-oriented leadership style Ability to partner closely with Account Executives and operational leadership to identify and develop new opportunities What You'll Do Provide leadership and operational oversight for the Senior Living Programs Lead, coach, and develop clinical and support teams while promoting accountability, engagement, and retention Build and maintain strong relationships with senior living communities and referral partners Serve as a key point of contact for facility partners and address clinical, operational, and service-related concerns Participate in case conferences, care coordination meetings, and partner meetings Monitor patient care, service delivery, staffing, and program performance Support appropriate nursing and therapy utilization and collaborate with clinical leadership to improve outcomes Identify opportunities to strengthen existing partnerships and expand program services Collaborate with Account Executives on referral development and program growth Track key program metrics and work with leadership to achieve clinical, operational, and growth goals Identify opportunities for process improvement, increased efficiency, and enhanced patient and partner satisfaction Support the continued development and expansion of Preferred Care Home Health's Senior Living Programs Why Join Preferred Care Home Health?
