Location:
Brevard County Area, FL A well-established Skilled Nursing Facility in the Brevard County market is seeking an experienced Hospital Liaison to drive referral growth and strengthen hospital partnerships. This is a full-time, permanent role focused on developing and maintaining relationships with acute care hospitals, discharge planners, and case managers to increase admissions and maintain census stability. Key Responsibilities Serve as the primary liaison between hospitals and the Skilled Nursing Facility Build and maintain strong referral relationships with discharge planners, case managers, and hospital leadership Conduct clinical referral reviews in collaboration with facility leadership Facilitate smooth transitions from acute care to post-acute placement Track referral activity, conversion rates, and market trends Represent the facility professionally in hospital settings Support census growth and payer mix strategy Required Qualifications Minimum 2+ years of experience as a Hospital Liaison or Clinical Liaison in a SNF or post-acute setting required Demonstrated success generating and converting hospital referrals Strong understanding of Medicare, Medicare Advantage, and managed care processes Established relationships within the Orlando-area hospital market strongly preferred Ability to work independently and manage referral pipelines Clinical background (RN, LPN, or Social Work) is a plus but not required.
