•VNA Home Health | Vero Beach, Florida Full-Time | RN Clinical Leadership VNA Home Health is seeking an experienced Home Health Registered Nurse to join our clinical leadership team as a Patient Care Manager (PCM). The Patient Care Manager provides day-to-day clinical and operational oversight for an assigned Home Health patient population and interdisciplinary field team. This role is ideal for an experienced Home Health RN who understands the complexities of managing an episode of care and enjoys developing clinicians, improving patient outcomes, solving operational challenges, and helping field teams deliver exceptional care in the home. What You'll Do As a Home Health Patient Care Manager, you will: Provide daily clinical leadership, coaching, and support to Home Health RNs, LPNs, therapists, aides, and other interdisciplinary team members. Maintain oversight of assigned patient caseloads, acuity, clinical risks, and continuity of care. Review and support Start of Care, Resumption of Care, recertification, discharge, OASIS, and episode-management workflows. Provide clinical oversight of the 485/Plan of Care, including disciplines, frequencies, interventions, goals, orders, and changes in condition. Monitor high-risk patients and partner with clinicians to prevent avoidable emergency department visits and hospitalizations. Respond to clinical escalations and help field clinicians make timely, appropriate patient-care decisions. Monitor visit completion, missed visits, schedule exceptions, outstanding orders, documentation, and HCHB clinical workflow. Partner with Home Health QA to resolve clinical and documentation concerns and reduce recurring errors. Work closely with Scheduling/Workforce Coordination to balance caseload, patient acuity, geography, clinician competency, productivity, and capacity. Monitor clinician productivity and help remove barriers that interfere with efficient, high-quality patient care. Coach clinicians on clinical practice, documentation, OASIS, Plan of Care execution, time management, productivity, and patient experience. Ensure changes in patient condition are appropriately assessed, communicated, documented, and incorporated into the Plan of Care. Participate in QAPI, clinical performance improvement, regulatory readiness, and Home Health operational initiatives. Support timely patient access and the continued growth of VNA Home Health while maintaining quality, safety, and regulatory compliance. What Success Looks Like Successful Patient Care Managers create an environment where patients receive coordinated care, clinicians are supported and accountable, and problems are addressed before they become larger issues. Performance is focused on patient outcomes, avoidable hospitalization reduction, timely and accurate Plan of Care execution, visit reliability, caseload management, documentation and QA performance, clinician productivity, workflow timeliness, patient experience, and team development. What We're Looking For We are seeking a Home Health RN who is both clinically strong and an effective people leader. You should be comfortable moving between patient-specific clinical issues, clinician coaching, HCHB workflow, performance data, and day-to-day operational problem-solving.

Qualifications include:

Current, unrestricted Florida Registered Nurse license. Strong professional nursing experience with Home Health experience strongly preferred. Previous Home Health case management, supervisory, preceptor, team-lead, or clinical leadership experience preferred. Working knowledge of Medicare Home Health requirements, OASIS, comprehensive assessments, 485/Plans of Care, orders, recertification, interdisciplinary care, and Home Health documentation standards. Experience with Homecare Homebase (HCHB) strongly preferred. Strong clinical judgment and ability to recognize and appropriately escalate changes in condition. Demonstrated ability to coach clinicians, establish expectations, provide constructive feedback, and drive accountability. Strong organization, communication, prioritization, and problem-solving skills. Ability to use clinical and operational data to identify trends and improve performance. Valid driver's license, reliable transportation, and ability to meet VNA driving requirements.
