Full-Time About Physicians Services Group Physicians Services Group (PSG) is redefining how medical care is delivered within skilled nursing and post-acute care facilities. We partner with healthcare centers to provide high-quality, continuous, and integrated clinical services that improve patient outcomes while supporting efficient facility operations. We are looking for an organized, dependable, and patient-focused Patient Care Coordinator (PCC) to join our growing team. The Role The Patient Care Coordinator is an essential part of the PSG clinical team and works closely with our physicians and advanced practice providers. In this role, you will serve as a key connection between patients, providers, facility staff, families, and PSG , helping ensure important patient care needs are communicated, documented, and addressed efficiently. This position is a strong fit for someone with healthcare experience who enjoys coordinating care, staying organized, solving problems, and supporting both providers and patients. What You'll Do Patient Care Coordination Coordinate daily patient care needs, provider visits, admissions, discharges, and transitions of care. Gather and organize clinical information, reports, referrals, and resources needed by providers. Facilitate communication between providers, patients, families, facility teams, and healthcare partners. Help ensure patient care documentation and follow-up are completed in a timely manner. Provider & Facility Support Serve as a key point of coordination between PSG providers and facility teams. Help providers work efficiently by anticipating needs, organizing information, and managing assigned workflows. Coordinate with nursing, administration, therapy, social services, and other facility departments. Identify and appropriately escalate patient care or workflow concerns. Documentation & Systems Maintain accurate patient and insurance information within PSG systems. Ensure required documentation is complete, organized, and available for provider review. Identify missing or incomplete information and coordinate necessary follow-up. Maintain patient confidentiality and follow all HIPAA requirements. Scheduling & Operational Support Maintain provider schedules and coordinate changes based on patient and facility needs. Assist with referrals, prior authorizations, DME, prescriptions, and other care-related workflows. Prioritize daily responsibilities to support provider productivity and patient care. Identify opportunities to improve workflows and escalate concerns when appropriate. What We're Looking For High school diploma or equivalent required. Previous healthcare experience preferred. Experience in a skilled nursing facility, assisted living community, physician practice, hospital, or similar healthcare environment is a plus. Experience working with electronic medical records (EMR/EHR) preferred. Familiarity with medical terminology and healthcare workflows preferred. Strong organizational and communication skills. Ability to manage multiple priorities in a fast-paced healthcare environment. Strong attention to detail and follow-through. Ability to work independently while collaborating effectively with providers and facility teams. Professional, reliable, and comfortable handling confidential patient information. Pay:
