Patient Care Manager — Key Responsibilities Provide clinical supervision: Ensure patient care aligns with professional standards, agency policies, and legal and regulatory requirements. Coordinate patient care: Plan, organize, and monitor services across the patient's episode of care. Supervise clinical personnel: Ensure services are delivered appropriately, consistently, and on schedule. Communicate effectively: Maintain strong communication with patients, caregivers, referral sources, and both field and office staff. Drive quality outcomes: Meet departmental goals and contribute to quality improvement initiatives. Model excellence: Demonstrate professionalism, service excellence, and organizational values in daily work. Uphold compliance: Complete required training, report accurately, and cooperate with audits and investigations. Promote safety: Follow and reinforce safety protocols to maintain a safe work environment. Develop your team: Coach staff continuously and encourage participation in continuing education. Patient Care Manager Qualifications Registered nurse with current licensure to practice nursing in the practicing state. Previous experience in home care setting with two years management or supervisory experience, preferred. Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
