Job Description
The Beginning Home Care is seeking a compassionate, organized, and dependable Care Coordinator to join our growing team. The Care Coordinator plays a key role in ensuring that clients receive exceptional care by coordinating caregiver schedules, maintaining communication with clients and families, and supporting day-to-day office operations. Responsibilities Assist with coordinating and managing caregiver schedules to ensure all client shifts are covered. Match caregivers with clients based on care needs, skills, and availability. Communicate professionally with clients, family members, caregivers, and referral sources. Respond promptly to call-offs and staffing emergencies, be ready to fill in any call-offs. Cover emergency shift for clients incase a permanent caregiver is not readily available Conduct client and caregiver follow-up calls to ensure satisfaction with services. Assist with new client admissions and caregiver onboarding. Maintain accurate client and employee records. Perform supervisory visits and document client care updates. Ensure compliance with company policies and Illinois home care regulations. Assist with other administrative duties as needed. Support recruiting efforts by screening applicants and scheduling interviews. Is self-reliant and plans appropriately. Qualifications High school diploma or GED (Associate's or Bachelor's degree preferred). At least 1 year of experience in home care, healthcare, scheduling, or care coordination. Self reliant and self starter Excellent communication and customer service skills. Strong organizational, multitasking, and problem-solving abilities. Proficiency with Microsoft Office (Word, Excel, Outlook). Ability to work independently and as part of a team. Valid driver's license and reliable transportation. Ability to pass a background check.
