Job Description

Transitions Care is seeking a Director of Clinical Operations RN / Hospice to lead and oversee hospice clinical services. In this role, you will manage daily operations, ensure regulatory compliance, and drive high-quality, patient- and family-centered care. You will supervise and mentor an interdisciplinary team, support complex clinical decision-making, and optimize workflows and staffing. The Director partners with leadership on quality initiatives, program growth, and clinical excellence, fostering a compassionate, mission-driven culture focused on dignity, comfort, and holistic end-of-life care. Responsibilities Oversee daily hospice clinical operations across service locations Ensure compliance with hospice regulations, policies, and quality standards Lead, mentor, and develop interdisciplinary clinical teams Collaborate with medical directors and providers on care plans Monitor clinical outcomes, patient satisfaction, and quality metrics Optimize staffing, scheduling, and resource utilization Develop and implement clinical policies, procedures, and best practices Support complex case review and escalation of clinical concerns Partner with leadership on strategic growth and program development Foster a mission-driven, patient- and family-centered care culture Required Skills Hospice clinical management RN clinical practice Regulatory and compliance knowledge (CMS, Joint Commission) Interdisciplinary team leadership Quality improvement and outcomes measurement Staff scheduling and resource management Policy and procedure development Electronic health record (EHR) proficiency Palliative and end-of-life care planning Case review and clinical escalation
