Personal Care Services Coordinator (with Health Information Technology Focus) Robert S Martino MD Dune Acres, IN 46304 From $38,438 a year - Full-time From $38,438 a year - Full-time Requirements Bachelor's degree in a medical-related field and degree in Health Information Technology. Minimum of 2 years of experience in a personal care, home health, or medical support role. Responsibilities Coordinate and develop comprehensive personal patient care services in a home-based setting. Schedule and oversee medical appointments, including arranging in-home visits with healthcare providers. Administer prescribed medications according to medical plans. Oversee in-home nursing care, as well as physical and occupational therapy sessions. Monitor patient using telemonitoring technologies on a continuous, 24/7 basis. Provide hands-on personal care, including assistance with bathing, hygiene, and nutrition through meal planning and preparation. Collaborate closely with family members and external healthcare professionals to ensure continuity and quality of care. Maintain availability on a 24-hour, 7-days-a-week basis for urgent patient needs and intervention.
