In this role, you'll support scheduling, care coordination, preventive-care follow-up, and accurate documentation, allowing our clinicians to focus on what matters most—providing high-quality, compassionate care to older adults.
Schedule:
Full-Time What You'll Do Key Responsibilities Schedule and confirm wellness visits, follow-ups, home visits, and transition-of-care appointments Make reminder calls and send electronic notifications Coordinate specialty referrals and help ensure timely appointments Assist patients and caregivers with patient portal navigation Monitor preventive-care gaps, including screenings and immunizations Proactively outreach to patients to help close care gaps Track risk-adjustment diagnoses and flag follow-up needs for the clinical team Welcome patients, answer calls, and respond to inquiries with professionalism Relay messages promptly and support a positive patient experience Verify insurance and review intake paperwork Administer patient screening questionnaires Scan and accurately index labs, consult notes, and hospital or ED reports into the EHR Maintain accurate, up-to-date patient records Support EMR document management and efficient patient flow Coordinate with clinicians, medical assistants, and care coordinators Assist with meeting coordination, note-taking, and follow-up tasks Provide day-to-day administrative support to keep practice operations running smoothly What We Are Looking For Education and Experience High school diploma or equivalent required Medical assistant training or certification preferred Previous medical office or healthcare experience required Geriatric care experience strongly preferred Familiarity with electronic health record (EHR) systems Additional Skills and Attributes Strong organization, accuracy, and follow-through Excellent and compassionate communication skills Empathy and patience when working with older adults and caregivers Understanding of primary care, preventive care, and basic insurance processes or willingness to learn Current valid driver's license Spanish-speaking skills are a plus What You Can Expect A supportive team of clinicians, nurses, medical assistants, and care coordinators Respectful, team-oriented environment where your ideas and contributions are valued Opportunities to grow professionally and make a meaningful impact on patient care Competitive salary and comprehensive health benefits Life and disability insurance Flexible spending account with annual employer contribution Employee assistance program Paid time off, sick time, and holiday pay 401(k) plan If you're looking for a rewarding opportunity where your organizational skills and dedication can make a meaningful difference, we'd love to hear from you.
