Job Description
Mental Health Service Coordinator Full-Time | Monday-Friday | 8:00 a.m. - 4:30 p.m. Join Our Team! Welcome House is seeking a compassionate and motivated Mental Health Service Coordinator to provide case management, mental health support, and advocacy for individuals experiencing or at risk of homelessness. This position plays a critical role in helping participants achieve housing stability, improve mental wellness, and increase independence. Key Responsibilities Manage a caseload of approximately 15-20 participants . Conduct mental health assessments and develop individualized service plans. Provide case management, crisis intervention, and ongoing participant support. Connect participants with mental health, healthcare, substance use, employment, and community resources. Conduct regular home visits and community-based meetings. Maintain accurate documentation and ensure compliance with HUD and agency requirements. Collaborate with internal teams and community partners to coordinate participant care. Qualifications Bachelor's degree required; Master's degree and clinical licensure preferred. 3-5 years of experience in social services, behavioral health, or case management. Knowledge of trauma-informed care, motivational interviewing, crisis intervention, and community mental health resources. Strong communication, organization, documentation, and problem-solving skills. Ability to work independently while collaborating with multidisciplinary teams. Why Join Welcome House?
