The Clinical Liaison evaluates potential patient referrals, conducts clinical assessments to determine appropriateness for post-acute services, and facilitates a seamless admission process for patients requiring Long-Term Acute Care (LTAC), Skilled Nursing Facility (SNF), and Rehabilitation services. The Clinical Liaison acts as a trusted resource to referral sources, patients, and families by providing education regarding available services, coordinating care transitions, and ensuring timely communication throughout the referral and admission process. The Clinical Liaison collaborates closely with clinical staff, admissions personnel, case management teams, and leadership to promote quality patient outcomes and enhance the organization's reputation as a leader in post-acute healthcare services Education and/or
