JOB DESCRIPTION AND POSITION REQUIREMENTS
Scheduled Weekly Hours:
40 Work Hours Monday- Friday 800-1630.
JOB SUMMARY
The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and helps coordinate a plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members.MINIMUM QUALIFICATIONS
LMSW or higher licensure required, LCSW preferred Three (3) or more years hospital social work or protective services experience preferred Experience in psychosocial and therapeutic counseling preferred Working knowledge/experience in case management, payors and reimbursement Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity Basic understanding of Medicare, Medicaid, and Managed Care regulations Organized and efficient in establishing priorities and performing tasks within specific timeframes Self-motivated requires minimal supervision Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress Responsible for maintaining strict confidentiality of all verbal & written information and documents Familiarity with and appropriate use of community resourcesPHYSICAL DEMANDS
