Our programs offer customized rehabilitation tailored to the specific needs of those recovering from a stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation or orthopedic injury.
POSITION SUMMARY
The Patient Coordinator provides direct patient care and supports the therapeutic milieu under the guidance of the Chief Nursing Officer, working closely with nursing and clinical staff to maintain a safe, structured, and therapeutic environment. This role ensures continuous patient supervision, engagement, and appropriate role modeling while facilitating assigned group sessions and therapeutic activities. The Coordinator delivers high‑quality customer service, communicating effectively with interdisciplinary teams, referral sources, and families or designated support persons to share updates and address concerns. Additionally, the Coordinator oversees program safety requirements and provides staff orientation and mentorship on crisis prevention, de‑escalation techniques, programming schedules, and behavioral management protocols.
ESSENTIAL FUNCTIONS
To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation. Understands patient treatment plans and guides staff in responding appropriately to treatment interventions, including training on effective de‑escalation techniques. Consistently maintains a trauma-informed and patient centered approach, using DBT and other therapeutic skills to engage with patients. Encourages patients to engage in healthy behaviors, including participation in groups and performing activities of daily living. Employs principles of communication, interviewing techniques, problem-solving and crisis intervention when performing psychotherapeutic interventions based on plan of care. Oversee the safety and stability of the milieu by performing patient checks, monitoring safety risks and contraband, and assessing changes in patient condition. Demonstrates strong milieu management skills that foster a positive and therapeutic patient experience. Adheres to policies and procedures that ensure a safe, clean, and therapeutic environment. Reports all patient medical changes promptly to the Registered Nurse. Maintains strict confidentiality and consistently complies with HIPAA regulations. Conducts safety checks and participates in safety codes in collaboration with Environmental Care leadership to support a secure environment. Attends supervisory meetings, in‑services, and scheduled trainings, maintaining current knowledge of fire, safety, and disaster procedures. Facilitates or cofacilitates psychoeducational and therapeutic activity groups using approved materials and trains BHTs to deliver the same content effectively. Provides patient observations and feedback to the interdisciplinary treatment team to support the development and modification of treatment plans. Serves as a preceptor for new clinical staff and BHTs, providing training on program schedules and the behavioral management system used across all groups. Access to and/or works with sensitive and/or confidential information. Exhibit a comprehensive understanding of healthcare regulatory and compliance (e.g., HIPAA). Skilled in the application of policies and procedures.
KNOWLEDGE, SKILLS

QUALIFICATIONS
High School Diploma required. Associate's degree in psychology, social work, or related field preferred. Two years of experience in a psychiatric healthcare setting required. CPR certification required within 30 days of employment and prior to any patient contact. De-escalation certification required within 30 days of employment and prior to any patient contact. Trauma Informed Care training required within 30 days of employment and prior to any patient contact. Dialectical Behavior Therapy (DBT) training required within 90 days of employment.
