Healthcare Regional Manager About Carease Health Carease is a nationwide preventative healthcare company focused on reducing hospitalizations. We are dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care. Carease Health partners with Skilled Nursing Facilities (SNFs) to deliver high-quality preventative care services, helping reduce hospitalizations, manage chronic conditions, and improve resident outcomes. Our model brings consistency, innovation, and collaboration to the skilled nursing environment through services like Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). We're seeking a motivated and experienced Regional Manager to ensure smooth delivery of our services across a group of SNFs, support on-site staff, and maintain strong partnerships at the facility level.
Job Responsibilities:
Oversee Service Delivery:
Ensure successful implementation and day-to-day operation of Carease's preventative care programs within assigned skilled nursing facilities.
Build Strong Relationships:
Act as the primary point of contact for facility leadership, including Directors of Nursing (DONs) and Administrators, to ensure alignment and engagement with Carease's services.
Ensure Regulatory Compliance:
Oversee adherence to all program protocols (RPM, CCM, CoCM), ensuring compliance with CMS guidelines and company standards.
Support On-Site Teams:
Help integrate Carease providers, care companions, and CNAs into the facility workflow, promoting a cohesive, collaborative care environment.
Conduct Facility Visits:
Travel regularly to facilities to monitor performance, resolve issues, and reinforce best practices.
Lead Monthly Strategy Meetings:
Review key metrics, address facility concerns, and align on upcoming goals and initiatives.
Resolve Technical Issues:
Coordinate and escalate any tech-related concerns promptly to avoid service disruption.
Job Qualifications:
Experience in Skilled Nursing Facilities (SNFs) is strongly preferred. Prior multi-site management or regional leadership experience is a plus. Strong interpersonal and problem-solving skills Ability to educate staff and give workshops Willingness to travel regularly. Ability to work independently while staying highly aligned with corporate objectives. Degree in healthcare related field preferred

$80,000.00 - $100,000.00 per year
