RN or LPN Join Our Team! The Center at Blue Hills is seeking a dedicated and detail-oriented MDS Nurse / MDS Coordinator to join our skilled nursing team in Stoughton, Massachusetts. This is an excellent opportunity for an experienced RN or LPN who is passionate about resident care, clinical documentation, and working collaboratively with an interdisciplinary team. The MDS Nurse plays a critical role in ensuring accurate assessments, comprehensive care planning, regulatory compliance, and quality outcomes for our residents. Responsibilities Coordinate and complete MDS assessments accurately and within required regulatory timeframes. Review resident medical records and clinical documentation to ensure accurate MDS coding and supporting documentation. Collaborate with nursing, rehabilitation, dietary, social services, activities, and other members of the interdisciplinary team. Participate in care plan meetings and assist with the development and updating of comprehensive resident care plans. Ensure resident assessments accurately reflect each resident's clinical condition, functional status, and care needs. Monitor MDS schedules and ensure timely completion and submission of required assessments. Identify documentation discrepancies and collaborate with the appropriate departments to resolve them. Monitor changes in resident condition and ensure appropriate documentation and assessments are completed. Maintain accurate and organized MDS-related records. Assist with regulatory compliance and survey readiness. Maintain confidentiality and comply with HIPAA and facility policies. Support quality improvement initiatives and other clinical projects as needed. Qualifications Current RN or LPN license in the Commonwealth of Massachusetts . Previous MDS experience strongly preferred. Skilled nursing or long-term care experience preferred. Knowledge of the CMS MDS assessment process and long-term care regulations. Strong clinical assessment, organizational, and critical-thinking skills. Excellent written and verbal communication skills. Ability to work effectively with an interdisciplinary team. Strong attention to detail and ability to meet deadlines. Experience with electronic medical records preferred. RAC-CT or other MDS certification is a plus.
