Duties Coordinate daily medical activities, including patient assessments, medication administration, and treatment plans. Maintain accurate and up-to-date medical documentation using electronic health record (EHR) systems such as Epic or Cerner. Assist with resident personal care, including hygiene support, vital signs monitoring, tube feeding, catheter care, and mobility assistance. Ensure compliance with HIPAA regulations by maintaining strict confidentiality of patient records and medical information. Collaborate with interdisciplinary teams to develop individualized care plans tailored to residents' needs, including dementia and memory care. Support staff training on medical procedures such as first aid, CPR, basic life support (BLS), and emergency response protocols. Manage medical supplies inventory and ensure proper handling of heavy lifting tasks safely within the facility. Facilitate communication between caregivers, families, and healthcare providers to ensure seamless patient care delivery. Qualifications Proven experience working in assisted living facilities, nursing homes, hospitals, or home & community care environments. Strong knowledge of physiology, anatomy, medical terminology, and senior/dementia/memory care practices. Certified in first aid and CPR; additional certifications in hospice or Alzheimer's care are a plus. Familiarity with EMR/EHR systems such as Epic or Cerner for accurate documentation of medical records. Experience working with individuals with developmental disabilities or disabilities-related health needs. Knowledge of state healthcare regulations and maintaining compliance with HIPAA standards. Ability to perform manual handling tasks safely and lift heavy objects when necessary. Excellent communication skills for caregiver coordination and resident personal care assistance. Valid license from the State Board of Nursing or relevant healthcare authority preferred.
