The ideal candidate will possess extensive clinical expertise, excellent communication skills, and a passion for supporting individuals through their recovery journey. As Assessment Coordinator, you will oversee patient assessments, coordinate multidisciplinary evaluations, and ensure accurate documentation aligned with clinical standards. Duties Conduct comprehensive mental health assessments, including DSM-5 diagnostic criteria, to determine patient needs and treatment planning. Coordinate intake procedures for new patients, ensuring smooth transitions into the program while maintaining confidentiality in accordance with HIPAA regulations. Utilize clinical tools such as cognitive behavioral therapy (CBT) techniques and crisis intervention risk assessments to evaluate patient stability and acuity. Collaborate with multidisciplinary teams—including social workers, therapists, and medical staff—to develop individualized treatment plans based on thorough assessments. Maintain accurate medical documentation and electronic health records (EHR), adhering to clinical documentation standards and ICD coding practices. Perform triage assessments for high-risk populations, including those with co-occurring disorders or substance use challenges, ensuring timely intervention. Provide ongoing case management support by monitoring patient progress through data collection, behavioral health evaluations, and follow-up assessments. Requirements Bachelor's degree in social work, psychology, counseling, or a related field; advanced degrees preferred. Proven experience working with individuals with mental health conditions, substance use disorders, or developmental disabilities. Strong knowledge of DSM-5 criteria, ICD coding (ICD-10/ICD-9), and CPT coding for clinical documentation. Expertise in crisis management techniques such as de-escalation strategies and behavioral therapy interventions. Familiarity with trauma-informed care approaches including PTSD care, grief counseling, and group therapy modalities. Experience working within acute care settings or hospital environments is highly desirable. Excellent communication skills to effectively engage clients and collaborate with multidisciplinary teams while maintaining patient confidentiality under HIPAA standards. Proficiency in electronic health records (EHR) management and medical records documentation practices.
