POSITION TITLE
Outpatient Services Clinician (BSW) or Supports & Services Coordinator
LOCATION
Alger or Luce County (This position will be responsible for coverage in both counties. The home location for the position will be determined by the successful applicant)
POSITION STATUS
80 hours biweekly / Regular Full-time, Salary / Exempt, Nonunion
SALARY RANGE
Pay rate dependent on Position, $24.61/hr---$31.99/hr LBSW pay scale, $23.26/hr---$30.24/hr LLBSW pay scale, $20.31/hr---$26.39/hr Supports & Services Coordinator, Plus full-time benefits
RESPONSIBILITIES
This position could provide case management and/or Supports Coordination services to a variety of clientele including adults, adolescents, children and families requesting services. Responsibilities include developing treatment plans using the person centered process, providing case management/supports coordination, and making appropriate referrals. Responsibilities also include monitoring treatment plans to assess recipient's progress and readjusting as appropriate, carrying an active caseload, preparing case histories and needs assessments, follow-up, and aftercare. An employee in this class may be assigned specialization areas or special populations such as adult, child or adolescent. Daytime crisis intervention, groups or family meetings may be assigned.
QUALIFICATIONS
For Outpatient Services Clinician - State of Michigan Licensed Bachelors of Social Work (LBSW) or State of Michigan Limited Licensed Bachelors of Social Work (LLBSW). For Supports & Services Coordinator - Possession of a Bachelor's Degree in a Human Services Mental Health field (Such as Psychology, Sociology, Criminal Justice, or similar nature) and one year of experience working with persons with mental illness.
