Instead of carrying your own full caseload, you'll become a mentor, coach, and trusted resource for clinicians serving more than 1,700 home health clients across our 21-county Twin Cities service area. You'll help therapists, nurses, and home health aides navigate complex situations, improve outcomes, and deliver exceptional care where patients want to be most—home. Qualifications Required Active Minnesota PT or OT license Previous Home Health experience Strong understanding of Medicare regulations, OASIS, and home health compliance Strong clinical decision-making and communication skills Ability to thrive in a fast-paced, collaborative environment Preferred Leadership, supervisory, preceptor, or mentoring experience Experience with EMR systems and electronic clinical documentation Passion for developing and supporting clinicians Great Therapy Leaders Leave a Legacy You can only impact so many patients yourself.
