Job Description

Methodist Hospital is looking to hire a RN Care Manager to join our Care Management team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
The RN Care Manager provides patient-centered care management services to patients and families within both the Emergency Center and inpatient settings. This position serves as an integral member of the interdisciplinary healthcare team and will be responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, patient and patient's external support system (as applicable). Within the context of an interdisciplinary team, the RN Care Manager will participate in progressing patient towards discharge/next level of care by identifying barriers and implementing appropriate solutions/interventions. The RN Care Manager will work with patients and patients' external support system to secure appropriate and safe transition plans in keeping with shared goals of care. The RN Care Manager will consistently and thoughtfully apply nursing values and continuous quality improvement in daily work.
Work Schedule:
Block schedule Monday-Friday Rotating schedule to include every 6th weekend
Required Qualifications:
Current Minnesota Registered Nurse (RN) license Bachelor's Degree in Nursing strongly preferred; would consider Associate's Degree in Nursing with commensurate experience. Minimum of one year hospital-based med-surg nursing experience preferred. Prior experience in any of the following areas strongly preferred: utilization management, case management, home care setup, discharge planning.
