PREVENTIVE
HEALTH COORDINATOR RESPONSIBILITY AND AUTHORITY
The Preventive Health Coordinator will assume functions delegated by the Aging Department Head to supports program development and provided direction to instructors for the implementation of evidence based programs in both community and in home virtual settings. The Coordinator will lead training sessions for instructors, teach proper exercise techniques, principles, and procedures, and lead wellness, health and education programs
EXAMPLES OF DUTIES
- Assists in the development of evidence-based programs that includes components of exercise and education to promote strength, balance, endurance, and flexibility that will promote health to include the prevention of falls;
- Assists in development of educational components of classes in accordance with current best practices in wellness prevention which are appropriate for clients of all levels of cognitive ability;
- Makes evidence-based recommendations for client assessment, including appropriate tools, frequency and evaluation methods.
- Visits class locations to audit volunteer and staff program delivery. measure and collect initial client baseline data and ongoing client outcomes, monitor equipment inventory, status, safety and storage;
- Completes administrative record-keeping including: tracking of client participation, equipment inventory tracking, reports for program delivery and outcomes, and of documentation completion;
- Participates in external partnership committees as directed by Aging Department Head;
- Accountable for being knowledgeable of all policies and procedures changes to wellness programs adopted by the Area Agency on Aging and not limited to CDSMP, DSMP, MOB, BINGOCIZE, and MASTERY;
- Responsible for maintain up to date licensing requirements for all wellness programs adopted by the Area Agency on Aging and not limited to CDSMP, DSMP, MOB, BINGOCIZE, and MASTERY;
- Responsible for budget of Title IID funding as it pertains to wellness prevention;
- Performs any other duties assigned as required by the Executive Director, Director of Senior Services, Assistant Director of Senior Services and/or Aging Department Head
QUALIFICATIONS A
minimum of a Bachelor's Degree in related human services/social service or health oriented field or appropriate work experience substitute.
