Case Manager / Care Coordinator (Self-Directed Services) Claremont, NH | Full-time | Flexible schedule Fast Switch is seeking a Case Manager / Care Coordinator to help individuals and families organize the services and resources they need. You'll work directly with the people you support, build relationships with providers and community partners, and help keep plans and budgets on track. What you'll do Coordinate services for a caseload of individuals and families Develop and monitor support plans and budgets Help people navigate Medicaid, Medicare, Social Security, and community resources Arrange assessments, evaluations, and other services Work with families, providers, and team members to resolve concerns Maintain accurate records, service agreements, and required documentation Meet with individuals locally and participate in an on-call rotation What you'll bring At least one year of experience in human services, case management, or care coordination Strong communication, organization, and documentation skills The ability to manage your caseload and work independently Proficiency with Microsoft Office A valid driver's license and reliable transportation A bachelor's degree in Human Services, Social Work, or a related field is preferred, but relevant experience will be considered.
