Job Description

Description Intake Coordinator The Intake Coordinator is responsible for providing excellent customer service and a welcoming environment for clients. The Intake Coordinator is the first point of contact for clients upon admission to the residential facility and will ensure an efficient admission process while interacting with clients, family, referrals, and guests.
Essential Job Functions:
Works closely with Admissions to ensure smooth landing of clients, including time of arrival, pertinent information and contact with external stakeholders. Completes admission forms with the responsible party, family member, power of attorney or client upon admission Update admissions communication on Teams platform to allow for transparency and collaboration of all departments involved Completes all required admission forms within the electronic medical record software Attend required trainings and meetings as directed Creates, maintains and stock supplies client binders, curriculum, and materials Supports Health Services as needed including coordination with providers for additional needs of admissions as they arise Supports with client engagement and appointment setting Completes client chart audits and updates appropriate staff when completed with accuracy Assists and supports various care teams to ensure best company optics; including but not limited to, coordination of appointments brought forth upon admission, communication of immediate needs of clients (i.e. FMLA, court, etc.), communication of relevant information learned upon admission to appropriate parties including care teams Capable of tracking, monitoring, and reporting clerical errors for admissions and electronic medical record system . Creates and maintains a safe, professional, and excellent client experience Takes ownership of the admissions building and processes to create an organized and welcoming environment. Other duties as assigned
Minimum Qualifications:
Minimum age requirement of 18 High school diploma or equivalent with at least 1-year prior experience preferred Communicating effectively in written and oral form Time management skills and the ability to work on multiple projects simultaneously Initiative to take charge of unforeseen situations, solve problems and adjust to changes. Working knowledge of Microsoft Office products Satisfactory references from employers and/or professional peers Satisfactory criminal background check
Salary Range:
Generally starting at $19.00 an hour. The salary of the candidate(s) selected for this role will be set based on a variety of factors, including but not limited to, experience, education, specialty and training.
