Job Description

Home Care Clinical Team Manager, Full Time Monday-Friday, 8am-4:30pm Established in 1904, Moorestown Visiting Nurse Association is a home care community health organization serving Burlington, Camden, and Gloucester counties in New Jersey. We are one of the very few home care agencies qualified to provide a continuum of care for patients of all ages, no matter how serious the problem, no matter what stage of the illness. It is care we've been delivering to residents of South Jersey for more than 120 years, 24 hours a day. Our services, experience and exceptional people are what set us apart in the minds of our patients. We're looking for a passionate and experienced Clinical Team Manager that is a Physical Therapist or Occupational Therapist to join our growing home care team. This is a great opportunity to make a real impact on the lives of our clients and the clinicians who care for them. In this role, you will lead a team of home care clinicians (RN's, PT's and OT's), ensuring they have the support and resources they need to deliver the highest quality of care. You'll be the clinical expert, providing guidance, mentorship, and oversight to ensure all care plans are safe, effective, and compliant.
Responsibilities:
Provide clinical supervision and expertise, overseeing complex cases and ensuring adherence to best practices. Conduct performance evaluations and provide ongoing feedback to team members. Collaborate with other departments to ensure seamless care coordination. Ensure compliance with all state, federal, and company regulations.
Qualifications:
Current Physical Therapist or Occupational Therapist license. Minimum of 3 years of experience with a certified home care agency, with at least 1 year in a leadership or supervisory position. Strong knowledge of home care regulations and best practices. Excellent communication, problem-solving, and organizational skills. The ability to work independently and as part of a team.
