Job Description

Physical Therapist -Full-time or Part-time in Burlington, Gloucester or Camden County area. $5000 sign on bonus for full time. Established in 1904, Moorestown Visiting Nurse Association is a non-profit home care community health organization serving Burlington, Camden, and Gloucester counties in New Jersey. We are one of the very few home care agencies qualified to provide a continuum of care for patients of all ages, no matter how serious the problem, no matter what stage of the illness. It is care we've been delivering to residents of South Jersey for more than 120 years, 24 hours a day. Our services, experience and exceptional people are what set us apart in the minds of our patients. We are currently seeking Full or Part time Physical Therapists to care for home care patients in the Camden, Gloucester or Burlington County areas. The Physical Therapist provides clinical services to patients with functional limitations that affect their ability to carry out activities of daily living. The Physical Therapist provides direct care, support, and education in the home therapy program under the direction of the attending physician and in collaboration with the family, nursing case manager, home health aide, and any other disciplines. Assignments require considerable independence of professional judgment and are carried out in accordance with best practice standards and established policies, procedures, rules and regulations of the agency.
Essential Duties and Responsibilities:
Assess referred patients for rehab services. Develop and educate patients and their care givers on the plan of care. Thoroughly complete all required documentation. Possess excellent interpersonal, communication and time management skills. Willingness to travel throughout our service area.
Education and Experience:
Licensed as a Physical Therapist by the State of New Jersey. Graduate of a Physical Therapist education program. Certification for CPR. At least 2 years of recent experience.
