$50,000-$60,000 per year + Benefits Job Summary The Patient Care Coordinator manages an assigned caseload and ensures patients receive consistent, reliable home care services. This role involves coordinating caregiver coverage, monitoring authorized-hour utilization, communicating with patients, caregivers, MLTCs, and internal teams, and ensuring timely follow-up on service issues. Key Responsibilities Manage an assigned patient caseload and coordinate day-to-day services. Ensure patients have reliable caregiver coverage based on their schedules, needs, and plan of care. Address open shifts, call-outs, caregiver changes, and staffing issues. Monitor utilization and take action to resolve low utilization or service barriers. Communicate with patients, caregivers, MLTCs, payors, hospitals, and internal departments. Monitor hospitalized, on-hold, or inactive patients and coordinate timely resumption of services. Follow up on missed visits, uncovered hours, hospitalizations, and service interruptions. Coordinate with staffing, intake, nursing, compliance, payroll, and other departments to resolve issues. Maintain accurate patient schedules, records, reports, and documentation. Escalate urgent patient, caregiver, authorization, or service issues when necessary. Build strong relationships with patients and caregivers while maintaining professionalism and accountability. Meet departmental goals related to staffing, utilization, responsiveness, documentation, and patient satisfaction. Qualifications Strong verbal and written communication skills. Excellent organizational and time-management skills. Ability to manage a high-volume caseload and multitask. Strong problem-solving skills and attention to detail. Ability to work effectively in a fast-paced environment. Strong sense of urgency, ownership, and accountability. Comfortable communicating with patients, caregivers, MLTCs, hospitals, and internal teams. Comfortable using scheduling systems, reports, and patient management software. Previous experience in home care, healthcare coordination, customer service, scheduling, or a related field is preferred.
Pay:
$24.00 - $28.00 per hour
