Skilled Nursing Facility in Suffolk County is seeking an experienced Social Worker to join our team as Director of Department.
REQUIREMENT
NURSINGHOME EXPERIENCE
Come join our team! Director of Social Work/Case Manager for a beautiful skilled nursing facility. Knowledge of Medicare and Medicaid, MLTC, HMO Case Management, Discharge Planning are required. NYS License, knowledge of Long Term Care and subacute short term rehab environment also a must.Job Type:
