    • We are seeking a dedicated Mental Health Treatment Coordinator to plan and facilitate comprehensive care for our patients.
    This role is essential in bridging the gap between inpatient treatment and outpatient recovery, ensuring every patient receives a holistic, multidisciplinary approach to mental wellness.
    Key Responsibilities Treatment Planning:
    Develop, implement, and update individualized care plans in collaboration with a multidisciplinary clinical team.
    Therapeutic Intervention:
    Apply psychotherapeutic techniques in group settings and provide psychoeducation to patients and their families.
    Care Coordination:
    Act as a liaison with outpatient providers to ensure continuity of care during hospitalization and seamless transitions upon discharge.
    Family Engagement:
    Facilitate family participation in the treatment process to support long-term patient success.
    Safety & Compliance:
    Maintain a secure environment through strict adherence to patient safety protocols, confidentiality (HIPAA), and regulatory standards.
    Qualifications & Requirements Education:
    Bachelor's degree in Psychology required. Master's degree in Social Work (MSW) preferred.
    Licensure:
    Valid NYS Licensed Master Social Worker (LMSW) license is required if the candidate holds an MSW.
    Certifications:
    Current BLS certification and PMCS (Preventing and Managing Crisis Situations) training required.
    Experience:
    Strong knowledge and clinical understanding of working with mentally ill populations.
    Skills & Attributes Interpersonal Excellence:
    Ability to communicate effectively and build rapport with a wide variety of personalities and backgrounds.
    Clinical Accuracy:
    High attention to detail in monitoring patient progress and maintaining precise clinical documentation.
    Technical Proficiency:
    Basic computer skills and familiarity with electronic health records.
    Professionalism:
    Committed to quality assurance and maintaining a clean, therapeutic environment.
