Hybrid Salary:
$50,000 - $52,000 Liberty Resources is seeking an Adult Health Home Care Manager to support adults with medical, behavioral health, and social service needs through care coordination and community-based support.
Care Manager Responsibilities:
Assess needs and develop person-centered care plans Coordinate services with healthcare and community providers Connect members to medical, mental health, housing, and social service resources Conduct home and community visits Manage a caseload of approximately 30 individuals Participate in an on-call rotation
Care Manager Qualifications:
Bachelor's degree + 2 years of relevant human services experience; or Master's degree + 1 year of relevant experience; or RN with 2 years of relevant experience Valid NYS driver's license and reliable transportation Care management/case management experience preferred Experience working with adults with mental health, substance use, developmental disabilities, or chronic health needs preferred
