Join the Serene Team! Serene Home Nursing Agency is a New York State licensed home care agency providing skilled nursing and home care services throughout Western Queens, Nassau County, and Suffolk County. We are seeking an experienced Patient Care Coordinator to coordinate patient schedules by matching insurance-authorized hours with qualified nursing staff. This is a fast-paced position requiring excellent communication, organization, and problem-solving skills. This is a non-clinical office position. Primary Responsibilities Coordinate insurance-authorized patient hours with qualified nursing staff. Fill open shifts quickly while maintaining continuity of patient care. Manage daily staffing schedules, call-outs, schedule changes, and coverage needs. Communicate professionally with patients, families, nurses, and internal departments. Monitor staffing to ensure authorized hours are utilized appropriately. Document scheduling activity accurately and maintain current patient records. Work closely with the clinical and administrative teams to resolve staffing challenges. Help ensure all cases remain properly staffed while meeting agency and payer requirements. Requirements Minimum 2 years of experience staffing home care patient cases (required). Excellent organizational, communication, and multitasking skills. Ability to work efficiently in a fast-paced office environment. Strong computer skills and the ability to learn multiple software programs. Experience with HHAeXchange is highly preferred. A degree in Communications is a plus, but no college degree is required . Preferred Qualifications Experience working in a Licensed Home Care Services Agency (LHCSA). Bilingual candidates are strongly encouraged to apply. Languages such as Spanish, Mandarin, Cantonese, Korean, Hindi, or Greek are a plus.
