JOB SUMMARY

: Responsible for the systematic approach to patient care, promoting professionalism in the case management process. The process includes assessing, planning, organizing, coordinating, implementing, monitoring and evaluating the services and resources needed to respond to an individual's healthcare needs.

ESSENTIAL JOB FUNCTIONS

: Completes all required admissions paperwork/packet Development of a plan that identifies short and long-term goals, involving the individual, support systems, inter professional collaboration and use of appropriate resources. Promotes effective, timely communication among healthcare team members participating in team meeting as indicated Conducts follow-up on any delays in treatment or reporting of results Monitors the effectiveness and timeliness of interventions and desired outcomes, and makes changes as needed in collaboration with the interdisciplinary team. Documents evaluations, case management interventions and utilization review needs Initiates and monitors the discharge planning process in collaboration with the interdisciplinary team. Performs other duties as may be required or assigned

JOB QUALIFICATIONS

: Bachelor's degree in Social Work, Human Services or a related field. A minimum of one year experience as Discharge Planner, Case Manager, or Social Worker required. Basic knowledge of community resources. Basic knowledge of case management process. Excellent clinical, written, verbal and interpersonal skills. Ability to liaison between key staff members. Ability to work independently and collaboratively with the interdisciplinary team patient/family Excellent computer skills
