Must be a Licensed Practical Nurse (LPN) or Medical Assistant (MA) in the State of Ohio. 2. Preferred experience in healthcare settings such as hospitals, primary care, care management, or similar health-related roles. 3. Must be trained in and comply with all OSHA and HIPAA regulations. Position Overview and Expectations 1. Participate in the care of patients, with a focus on those who are high-risk or chronically ill. 2. Provide transitional care and follow-up for patients discharged from the Emergency Department. 3. Assess and identify patient needs and coordinate referrals to home health care and other appropriate community resources. 4. Assist with the assessment and care planning for complex patient needs. 5. Perform clinical or clerical duties as assigned, skillfully and efficiently. 6. Demonstrate strong verbal and written communication skills. 7. Maintain professionalism and sound judgment in high-pressure or stressful situations. 8. Collaborate effectively as part of an interdisciplinary team. 9. Maintain patient confidentiality in accordance with HIPAA and organizational standards. 10. Adapt to changing patient needs and healthcare environments. 11. Perform other duties as assigned.
Location:
Administrative office in Green, Ohio with rotation to alternative Pioneer office sites. Physical Demands 1. Requires sitting for extended periods of time. 2. Some bending stretching, lifting, and stooping may be required. 3. Requires manual dexterity sufficient to operate office equipment including but not limited to telephone, keyboard, copier and fax machine. 4. Requires normal range of hearing and vision. 5. Requires the ability to communicate with patients, office personnel and the community.
