Indian Health Care Resource Center of Tulsa (IHCRC) is a nonprofit organization empowering the American Indian community through exceptional, culturally responsive healthcare. Our team-driven environment is rooted in community, respect, accountability, excellence, and stewardship, with a shared commitment to health promotion and disease prevention. We are seeking a dedicated and compassionate RN to join our organization as the Patient Care Coordinator. The Patient Care Coordinator (RN), is responsible for assessing patients and determining their level of need for medical assistance. The PCC RN assess patient condition and follow provider approved protocols and make disposition or escalation deciisions efficiently and accurately.
ESSENTIAL JOB FUNCTIONS
. Conduct assessment to meet patients' need for medical, social, and mental assistance. Schedule appointments as needed. Ensure accurate and timely documentation of calls and to return calls in a timely manner. All other responsibilities as directed by providers/nurse management and all other duties. Back up nurse for same day/walk in clinic as needed. Process patient prescription refill requests. Address chart notifications per medical providers. Adherence to triage guidelines for adult and pediatric patients. Competency in taking and recording vital signs. Ability to dispense medication to pediatric patients per triage guidelines. Asists with promoting IPMCS program. Ensures care teams are functioning to maximum efficency. Responds to short notice staffing changes when direct supervisor is out of the office. Coordinates improving Patient Care (IPC) model. High School diploma or equivalent. Completion of an accredited RN program. Current and valid Oklahoma RN license. 2+ years of clinical experience preferred. BLS CPR Certification.
