Job Description

This position is the organizational leader of the Care Management Program at CMH. The goal of Care Management at CMH is to assure that our team provides person centered care coordination to our high-risk patients with either or both complex medical needs and psychosocial needs while utilizing evidence based best practices in the field including population health strategies.
Job Requirements Knowledge of/Skill/Ability to:
Maintains the standards of Care Management excellence in accordance with principles set forth by the State Health Department, Columbia Memorial Hospital, and agency policies and procedures. Performs short-term crisis intervention and individual or family counseling when indicated. Able to supervise staff and manage crisis situations. Must demonstrate proficiency in project management and program development License/Certification Currently licensed RN with the Oregon State Board of Nursing or Social Worker licensed with the Oregon State Board of Social Work as an LCSW or
CSWA. LCSW
with supervisory permission preferred. Education and/or Experience Minimum Baccalaureate in Nursing (Master's preferred), or Masters in Social Work required. Must demonstrate a progressive supervisory history in patient care or Care Management. Must demonstrate proficiency in working with development of policies and with Microsoft products including Word, Excel, Power Point, Outlook.
