As a care manager for

your anticipated job duties will include, but not limited to,

Personal Care:

Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting. Help residents with mobility, transfers, and positioning to ensure their comfort and safety. Provide assistance with eating and hydration as needed.

Healthcare Support:

Monitor residents' health status and report any changes to the Nurse Manager or Charge Nurse.

Emotional and Social Support:

Engage with residents in meaningful activities and social interactions to promote their mental and emotional well-being. Provide companionship and emotional support, fostering a positive and caring environment.

Documentation and Communication:

Maintain accurate and up-to-date documentation of care provided to residents including but not limited communication with lead charge nurse daily. Communicate effectively and professionally with residents, families, and other healthcare team members regarding residents' care plans and needs.

Safety and Compliance:

Follow infection control procedures and maintain a clean and safe environment for residents. Adhere to all facility policies and protocols, as well as state and federal regulations governing senior living facilities. Any additional duties as assigned

Qualifications:

High school diploma or equivalent. Previous experience in a healthcare setting preferred Compassionate and patient-centered approach to caregiving. Strong communication and interpersonal skills. Ability to work effectively as part of a team and independently as needed. Direct Care Certificate will be a requirement.
