Job Description

Client Health & Wellness Assistant Client Health & Wellness Coordinator Assists in providing for the care needs of the program participants, including emergent and chronic medical needs. Provides administrative support to the Health and Wellness Department. 1) Administer prescribed medications to clients according to DHS guidelines and maintain related medical records in accordance with Acadia's policy. Verify identity of client receiving medication and record name of drug, dosage, and time of administration in the Electronic Medication Administration system. 2) Review all medical appointment summaries for the prior week to ensure a summary has been completed for each client medical appointment. Assist with scanning and charting the information in the client's medical chart 3) As is appropriate, take vital signs or observe client to detect response to specified types of medications and prepare report or notify Client Health and Wellness Coordinator or other designated personnel of unexpected reactions, with vital signs. 4) Contact all necessary parties when discovering or making a medication error, completing, and submitting required documentation according to Acadia policy. 5) Maintain the medication administration office, standing medication orders supply closet, the medical supply cabinet, keeping clean and well organized. Take inventory monthly and give the needed supply list to the appropriate personnel. Inventory and maintain all other supplies needed for medication administration (e.g., applesauce, spoons, med cups, etc.). 6) Monitor supplies and complete ordering for PPE. 7) Assists the Client Health and Wellness Coordinator to update spreadsheets for client narcotics, spreadsheets for routine wellness/preventative care medical visits, scan and file medical documents in charts, enter vaccine data into CaraSolva, alerting staff to red prompts in CaraSolva that need to be checked off, and other administrative tasks as assigned. 8) Maintains and reconciles all medications that are expired or discontinued. Determines when it is appropriate to return medications to the pharmacy and when it is appropriate to discard the medications per Acadia Policy. 9) Call the pharmacy to request refills for medications, or to seek guidance related to client medications, as needed; reconcile meds in CaraSolva's Pharmacy Waiting platform. 10) Receive weekly refills from pharmacy. 11) Make phone calls to clients' physician offices, as needed by direction of the Client Health and Wellness Coordinator. Follow up with appropriate parties on all client medication changes to ensure clear communication of all medication changes. 12) Initiate and respond timely to communication (emails, text messages, voice mails, face-to-face communications) from Client Health and Wellness Department and other staff involved in the medication administration process. 13) In keeping with manufacturer's guidelines, routine checks on medical devices (e.g., CPAP machines) used by clients to ensure cleanliness and functionality, and reports concerns to the client's case manager and the Client Health and Wellness Coordinator. 14) Assists with repairs and adjustments to adaptive equipment, durable medical equipment and personal care items. 15) Complete all documentation as needed on MAR system, Weekly Data Sheets 16) Monitor clients who become sick during day treatment hours at clinic and assist in making decisions about the need for a client to be returned to the residential program. 17) Take clients to medical appointments if needed. 18) Participate in med audits as directed by Client Health and Wellness Coordinator. 19) Work collaboratively with and in support of clinic leadership, residential leadership, and all direct care staff in both departments to ensure team cohesion and effective problem-solving as it pertains to medication administration procedures. 20) Maintain medical appointment calendar to reflect all new, changed or cancelled appointments, and communicate changes to appropriate staff. Assist the Client Health and Wellness Coordinator with other administrative tasks as assigned. 21) Provide direct care to clients in a competent and professional manner. 22) Assist in faxing OT, PT, speech and any other orders, collect faxes and assist with other administrative tasks as assigned. 23) Complete all required trainings 24) Attend and participate in weekly Client Health and Wellness meetings and any other meeting that attendance is requested. 25) Monitors and tracks medication administration training and insulin traning due dates for staff, sending out reminders and scheduling classes and reviews Determined by Client Health and Wellness Coordinator at the time of employment. The position is full-time. Determined by the President/CEO with input from Human Resources Director, and in accordance with Acadia personnel policies. This is a non-exempt position and utilizes the wage range that has been established for the position. Wage rates will be reviewed annually at the time of performance appraisal. 1. Must be able to perform the activities required to successfully complete the Red Cross certified courses in First Aid and CPR. 2. Must be able to independently perform client transfers in accordance with Acadia's policy and procedure for client transfers, including maximum assistance transfers. 3. Must be able to lift, push, pull and move a minimum of 50 lbs. 4. Physical tasks include walking, standing, and sitting for up to 8 hours. 1. Must be able to learn and utilize Acadia's electronic health record (E.H.R.) and electronic medication administration record (EMAR) systems. 2. Must have basic computer skills using Microsoft Word. 3. Must be proficient in verbal and written communication. 4. Must be able to learn and utilize Acadia's phone and email systems. 5. Must demonstrate the ability to work collaboratively with a diverse population of professionals and direct-care staff. Work is conducted in a shared office inside the Acadia NeuroRehab Outpatient Center facility, with tile covered concrete floor, inside Acadia's residential apartments or townhouse among clients living there and other staff, in fully integrated communities. All Acadia facilities are temperature controlled. Some work may be conducted in the larger community (e.g., medical facilities). Transporting clients in company vehicles in all types. Work is conducted inside residential apartments or townhouse in fully integrated communities, among clients living there and other staff, floors are laminate-covered concrete, stairs in townhouse are carpeted; and in a shared office in Acadia's clinic facility, floors are tile-covered concrete. All facilities are temperature controlled. Some work may be conducted in the larger community (e.g., medical facilities). Must be task-oriented, have impeccable attention-to-detail, and respond quickly and appropriately to situations in a demanding environment.
