Job Description

$7,000 signing bonus! Caring Hospice provides nursing care and emotional support to hospice patients both in their homes and in nursing facilities. We are deeply committed to a quality end of life experience for all our patients. The Patient Care Manager is responsible for the coordination of all clinical services (Planning, organizing, directing and supervising) provided to the hospice patient. The Patient Care Manager is a member of the interdisciplinary team. This position oversees the day to day clinical operation of hospice in conformance with the organization's mission, philosophy and goals and ensures that services conform to all applicable laws, rules and regulations.
Reports to:
Program Director Licensed as a Registered Nurse in PA.
Education:
A Registered Nurse with a Baccalaureate or higher degree preferred in nursing or other health related field. And two year's clinical experience with the last three (3) years in a home health agency, home care setting or health facility.
Experience:
Two (2) to four (4) years' experience in a patient care setting, acute care desirable OR two (2) to four (4) years' experience in a supervisory or administrative position in a hospice organization OR equivalent and demonstrates a strong knowledge of regulations
